BioNTech Cancer Vaccine Misses PFS Primary Endpoint in HNSCC Study

BioNTech has reported that AHEAD-MERIT, a randomised open-label Phase II/III trial of BNT113 plus pembrolizumab versus pembrolizumab alone in first-line PD-L1 CPS≥1, HPV16-positive unresectable recurrent or metastatic head and neck squamous cell carcinoma (HNSCC), did not meet its progression-free survival (PFS) primary endpoint, with no significant PFS difference between arms.

The independent data monitoring committee recommended the study continue as planned to read out the other primary endpoint, overall survival (OS), leaving the asset's pivotal-case status unresolved even though earlier ESMO 2024 Phase II data had shown a 40.0% unconfirmed objective response rate (ORR), 53.3% unconfirmed disease control rate (DCR), 3.9-month median PFS and 22.6-month median OS in a small 15-patient cohort.

BNT113 is an mRNA cancer vaccine encoding HPV16 E6 and E7, targeting a biologically attractive setting: HPV drives roughly one-third of HNSCC cases and about 90% of oropharyngeal cancers, yet no HPV-specific therapy is approved, so the strategic prize is a tumour-vaccine proof point rather than just another IO combination.
